29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems were rigged in such a manner that the employee could not free fall more than 6 feet (1.8 m) or contact a lower level.      a) Warehouse: The employer did not ensure that fall protection, such as, but not limited to,  the 3M Safelight LOPRO Shock Lanyard model 209712 used by employees when picking and stocking products from a three tiered racking system would prevent the employees from contacting a lower level in the event of a fall. The LOPRO Shock Lanyard was 6 feet long and had a shock absorber system that would allow the employee to fall to a lower level, on or about 6/21/17. Aruvil International Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(h)(2), which was contained in OSHA inspection number 1084331, citation number 1, item number 4 and was affirmed as a final order on 6/27/16 with respect to a workplace located at 6925 Sherman Avenue, Pennsauken, NJ 08110.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
