(a) Worksite at 22550 Barbour County Highway, Phillipi, WV: On or about June 20, 2017 employees were exposed to over pressurization and explosion hazards while conducting a tank draining process during the decommissioning of MP 85 odorant tanks.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers was not provided, used, and/or maintained: (a) Worksite at 22550 Barbour County Highway, Phillipi, WV: On or about June 20, 2017, a protective shield and/or barrier was not used during the opening of a valve on the MP 85 odorant tank, resulting in exposure to shrapnel and other flying debris from a ruptured vessel.
