29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:    The employer did not implement feasible administrative controls or engineering controls when employees who worked at the facility were subjected to sound exceeding those listed in Table G-16 were not utilized.    (a) A hammer forge operator employee was exposed to continuous noise levels at 498.8% of the allowable 8-hour time weighted average (TWA) permissible exposure limit (PEL) of 90 dBA. The equivalent level of 498.8% is approximately 101.6 dB. Sampling was performed for 398 minutes during one shift. Zero exposure was assumed for 82 minutes not sampled.     (b) A hammer forge operator employee was exposed to continuous noise at 1070% of the allowable 8-hour time weighted average (TWA) permissible exposure limit (PEL) of 90 dBA. The equivalent level of 1070% is approximately 107 dB. Sampling was performed for 478 minutes during one shift. Zero exposure was assumed for 2 minute not sampled.   (c) A hammer forge operator employee was exposed to continuous noise at 939.6% of the allowable 8-hour time weighted average (TWA) permissible exposure limit of 90 dBA. The equivalent level of 939.6% is approximately 106.1 dB. Sampling was performed for 484 minutes during one shift.     (d) A hammer forge operator employee was exposed to continuous noise at 725.6% of the allowable 8-hour time weighted average (TWA) permissible exposure limit of 90 dBA. The equivalent level of 725.6% is approximately 104.2 dB. Sampling was performed for 390 minutes during one shift. Zero exposure was assumed for 90 minutes during one shift.
