29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 21 June 2017, a roofer was moving materials from the ground up onto a second story roof without the use of a hard hat, exposing the worker to the hazard of being struck by falling materials, tools, and debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  On or about 21 June 2017, two roofers were applying the vapor barrier underlayment on a two story residence under construction without fall protection, exposing each to a fall hazard of ~24 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about 21 June 2017, two roofers accessed the roof of a residence under construction in which the extension ladder was not extended to at least 3 feet above the upper landing, exposing the roofers to a fall hazard of ~10 feet to the ground below.
