29 CFR 1910.145(c) (2) (i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:    a.  215 Oneta Street, Athens, GA - Employees were exposed to pinch point hazard.  Employees/Operators are working on the Drop Mill Lines where the rubber has drop down from the mixer and onto the rotating mill roller. The employees are exposed to pinch point area on the rotating mill.  The operators work in close proximity to the pinch point area. There were no signs/placards posted on the on the drop mill rollers and/or in the area alerting employees of pinch point areas on the mills, on or about 06/21/17.        a. Line #11 Drop Mill       b. Line #12 Drop Mill     b.  215 Oneta Street, Athens, GA - Employees were exposed to hot surface/rubber material. Employees/Operators are working on the Drop Mill Lines where the hot rubber (330 degrees F) has drop down from the mixer and onto the rotating mill roller. There were employees who access the area and/or walk in close proximity to the drop mill where the hot rubber is located. There were no signs/placards posted on the on the drop mill rollers and/or in the area alerting employees who access the area of the hot surface/material, on or about 06/21/17.        a. The company's written lockout/tagout specific procedures were found with the following deficiencies:    1.  All the energy source was not identified and/or listed on several machine specific procedures.    2.  The energy control procedures did not specifically outline the steps for placement of locks onto the multi-   energy source disconnect apparatus and how to relieve pressure from the energy source that was not identified      and/or listed.

Employees (maintenance) perform servicing and maintenance on various machinery and equipment which has multi-energy source, such as electrical, pneumatic, thermal and hydraulic. An internal audit was not performed to ensure de-energization procedures were being properly implemented, on or about 06/21/17.
