Operators placed their hands in the point of operation while performing die setting without utilizing hazardous energy control procedures. This exposes employees to caught-in and amputation hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) Cadet Steel at 1806 East 40th Avenue, Denver, CO 80205: On or before June 20, 2017 the employer did not provide adequate training on the energy control (lockout/tag out) program to ensure that all required elements of the program were understood by all authorized employees whose work operations were, or may be in an area where energy control procedures are utilized. This exposes employees to caught-in and amputation hazards. Abatement Note: 29 CFR 1910.147(c)(7)(iv) provides that the employer shall certify that employee training has been accomplished and is being kept up to date. Abatement Note: The training shall include the following: 1) Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. 2) Each affected employee shall be instructed on the purpose and use of the energy control procedure. 3) All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out or tagged out.

Operators place their hands within inches of the point of operation when rotating the bottom dye of the press brake. This hazard exposes employees to amputation hazards. (b) Cadet Steel at 1806 East 40th Ave. Denver, CO 80205: On or before June 20, 2017 the employer did not ensure the point of operation on the Accurshear, model 625012, shear machine was properly guarded to prevent employees from having any part of their body in the danger zone during operation. Operators place their hands within inches of the point of operation when placing metal sheets into the shear machine. This hazard exposes employees to amputation hazards.
