29 CFR 1926.753(d)(2)(i): Materials being hoisted were not rigged to prevent unintentional displacement: a) On or about June 20, 2017, a beam that had been hoisted upside down had been rendered and rigged so that it was not centered. Upon re-hoisting the beam it contacted an employee and/or a beam on which he was walking, causing the employee to fall approximately 14 feet.

29 CFR 1926.753(d)(2)(iii): All loads were not rigged by a qualified rigger: a) On or about June 20, 2017, suspended loads with employees working underneath the loads were not rigged by a qualified rigger. A load which had been rigged incorrectly (beam upside down) was rendered and rigged by an employee that was not a qualified rigger. Upon re-hoisting, the beam was off center and contacted an employee and/or the beam on which he was walking, causing the employee to fall approximately 14 feet.
