The Toyota forklift, model 7EBU415 was in service and being used by employees with broken seatbelts as well as chunks missing from the wheels. The Toyota forklift, model 7EBGT5 was being used with a broken seat belt.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage: Employees exposed to electrical shock hazards. The electrical panels were blocked by pallets of forklift batteries prohibiting employees from accessing the panels in a quick and safe manner.

29 CFR 1910.303(g)(2): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures: Employees were exposed to electrical shock hazards. Employees were exposed to live electrical parts in that an electrical outlet receptacle near the rendering line had a broken face plate. The outlet as well as the junction box next to it had been knocked loose from the pole in which they were attached.
