This was most recently documented at 204th St. & Maple St., Elkhorn, Nebraska on or about June 21st, 2017 when the employer had employees moving a rough terrain forklift on site without any form of training to do so.

This was most recently documented at the corner of 204th St. and Maple St., Elkhorn, Nebraska on or about June 21th, 2017 when the employees were using an extension cord on site that is missing the grounding prong.

Workers were observed utilizing damaged extension cords running from the power generator on site.  The employer is failing to properly protect the cords utilized on this job site from damage due to vehicle and forklift traffic through the site.  This was most recently documented at the corner of 204th St. and Maple St., Elkhorn, Nebraska on or about June 21th, 2017 when the employer had run over the extension cord being used to power tools with the rough terrain forklift resulting in damage to the extension cord.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:       The employer is failing to protect employees from electric shock hazards while utilizing electric extension cords during construction activities on site.  Workers were observed utilizing a cord that had no strain relief device to keep the cord from separating from the plug.  This was most recently documented at the corner of 204th St. and Maple St., Elkhorn, Nebraska on or about June 21th, 2017 when the employees were using an extension cord on site that had been pulled away from the plug exposing the inner wiring which could shock anyone utilizing the cord.

This was most recently documented at the corner of 204th St. and Maple St., Elkhorn, Nebraska on or about June 21th, 2017 when the employer had employees using a skill saw to cut sheathing for the roof of the commercial building being constructed at this job site.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     The employer is failing to provide employees with adequate and effective fall protection training prior to allowing them to work on a roof. Workers were observed installing sheathing on a residential style construction site (Bank) without utilizing fall protection.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  The workers were exposed to falls of over 14 feet to dirt and concrete below.  This was most recently documented at the corner of 204th St. and Maple St., Elkhorn, Nebraska on or about June 21th, 2017 when the employer had employees working on the roof of this residence without any form of protective system and had not provided any training to them.

This training would allow each employee to recognize the hazards of improperly using ladders in order to minimize these hazards.  The workers were exposed to falls of over 14 feet to dirt.  This was most recently documented at the corner of 204th St. and Maple St., Elkhorn, Nebraska on or about June 21th, 2017 when the employer had workers accessing the roof of the building being constructed on site by stepping on the top two steps of the ladder.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.       The employer is failing to ensure employees are protected from fall hazards when conducting installation of sheathing materials.  Employees were observed installing sheathing materials on a roof without utilizing fall protection.  The employees were exposed to falls of over 14 feet to dirt below.  This was most recently documented at 11829 S. 52nd St., Papillion, Nebraska on or about June 26th, 2017 when the employer had employees working on the roof of this building without any form of protective system and the workers were outside of the parameters for using the platform as fall protection.  Workers were observed farther outside of what protection a platform would provide in the event someone was to fall from on the roof.     R & M Equipt, L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1123059, citation number 1, item number 1 and was affirmed as a final order on December 27, 2016, with respect to a workplace located at 18650 N. HWS Cleveland Blvd., Elkhorn, Nebraska 68022.
