29 CFR 1910.119(f)(4): The employer did not follow safety rules for control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel. These safe work practices shall apply to employees and contractor employees. On or about February 21, 2017, at Blanchard Refining Company, LLC in Texas City, Texas; the employer failed to ensure its employees followed the safety work rules of the facility including but not limited to site security and control in the Oil Movement Process Area.
