29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Jobsite, 612 Elmbank Street, Pittsburgh, PA 15226: On or about May 31, 2017, employees working at the bottom of a trench that was approximately 8 feet 7 inches deep with vertical sidewalls were not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. The employees were exposed to an engulfment hazard while engaged in work at the bottom of the excavation.
