29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job site, materials and equipment to be made by competent persons designated by the employer: On or about June 21, 2017, the employers jobsite inspections were ineffective in that hard hat use was not enforced to protect from overhead hazards and ladders were not required to be used to protect against falls.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets. On or about June 21, 2017, workers were laying stone on the commercial project under construction up to approximately 12 feet high without the use of hard hats, exposing workers to the hazard of being struck by tools or materials.
