29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On 22 June 2017, four workers were performing framing activities with overhead work ongoing without the use of head protection, exposing the four workers to the hazard of being struck by falling tools, materials, and debris.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On 22 June 2017, a worker was performing framing activities with a pneumatic nail gun without using eye protection, exposing the worker to the hazard of being struck by flying particles or objects to the face and eye(s).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  On 22 June 2017, two workers were performing framing activities on a residence under construction without the use of fall protection, exposing the workers to fall hazards of approximately 10 feet 1 inch and 21 feet to the concrete foundation below.

29 CFR 1926.1053(b)(13): The top or step of a stepladder was used as a step.  On 22 June 2017, two framers used a 6-foot stepladder to access the top plate of the residence under construction by utilizing the top step of the ladder, exposing the workers to a fall hazard of approximately 10 feet 1 inch.
