29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 20 June 2017 and at times prior thereto, two employees without head protection were working at ground level and passing materials to two employees working on a higher level of a residence under construction, exposing the employees to the hazard of being struck in the head by falling materials, equipment and objects.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about 20 June 2017 and at times prior thereto, two employees without eye protection performing framing activities using pneumatic nail guns on the second floor of a residence under construction, exposing the employees to the hazard of being struck in the eyes by flying materials and debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about 20 June 2017 and at times prior thereto, two employees without fall protection were performing framing activities from the top cap of the first floor of a residence under construction, exposing the employees to the hazard of falling approximately 10 feet to the ground below.

On or about 20 June 2017 and at times prior thereto, two employees used the top step and top of a portable step ladder to access the second floor and top cap of the first floor of a residence under construction, exposing the employees to the hazard of falling approximately 6 feet to the ground below.
