29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:  a) On or about 6/16/17 at Skrlin Coating & Blasting Inc. located at 2901 Lockport Road, Niagara Falls, New York; an employee painting a tank in the paint shop fell approximately 6'-4" through a 21" x 40" opening in the top of the tank. The employee was also exposed to falling approximately 7'-6" off from the outer edges of the tank. There was no fall protection system in place.  NO ABATEMENT CERTIFICATION REQUIRED
