29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: (a) On June 15, 2017, the employer did not report within 8 hours the death of an employee who had passed from a prior work related in-patient hospitalization event. OSHA was not made aware of the employee passing until June 20th, 2017 during the opening conference of the investigation.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours: (a) On June 08, 2017, the employer did not report the in-patient hospitalization of an employee within the twenty-four hour requirement. An employee collapsed while working on the Laundry Sorting Line of the facility located at 1540 SO Enterprise, Springfield, MO., was taken and admitted to the hospital for medical treatment. The event was eventually reported to OSHA by a family relative on June 13, 2017.
