29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Bucyrus Blades, Inc. on or about June 16, 2017, at the address of 260 E. Beal Ave. Bucyrus, OH: The employer did not ensure that employees who were exposed to energized rotating parts during service and cleaning tasks on the Moline Multi Spindle Drill (machine #147) were adequately trained in the skills required for the safe application, usage, methods, and means to isolate and control potential hazardous energy sources. The employees tasked with this type of work on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing service work near the point of rotation.

Beal Ave. Bucyrus, OH: The employer did not ensure that machine specific energy control procedures were utilized to control the energization of the Moline Multi Spindle Drill (machine #147) during service and cleaning task(s). Employees entering the point of operation to remove and adjust tooling bit(s) and to perform clean out activities without the benefit of energy isolation were exposed to caught-in hazards.

Employees operating the drill press were exposed to rotating parts and caught-in hazards during blade fabrication operations.
