29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Construction Site: On or about June 22, 2017, the employer did not ensure that employee retained the requisite understanding or skill on the operation of the pump truck mounted boom controls. This condition exposed employee to electrical shock hazards.

29 CFR 1926.600(a)(6)(ii): For lines rated over 50 kV, a minimum of 10 feet plus 0.4 inch for each 1 kV over 50 kV, or twice the length of the line insulator was not maintained between the energized line and any part of the equipment or load:  Construction Site: On or about June 22, 2017, the employee was operating the pump truck mounted boom controls without having a view of boom tip. The Supervisor (spotter) allowed the boom tip to get within 10 feet of the energized electrical line. This condition exposed employee to electrical shock hazards.
