On or about June 21, 2017, training for powered industrial truck operators did not consist of a combination of formal instruction, practical training and evaluation of the operator's performance in the workplace. Employees were exposed to the hazard of being stuck by falling loads.

29 CFR 1910.1200(f)(8): The employer did not ensure that portable containers into which hazardous chemicals were transferred from labeled containers and which were not intended for the immediate use of the person doing the transfer, were properly labeled. On or about June 21, 2017, shop area, gallon containers were not properly labeled. The product was not used within one shift. Employees were exposed to the hazard of respiratory illness and/or eye and skin irritation.
