29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 21, 2017, employees were exposed to a nine foot fall hazard while installing decking and other construction fall hazards and the employer did not ensure the use of a suitable fall protection system.  a. On the west side of the second floor deck between the buildings there was no fall protection in use.  b. On the east side of the second floor deck between the buildings there was no fall protection in use.  c. The opening around the stairs were not protected with a suitable guardrail or another form of fall protection.  d. The doorways on the second and third floors were not protected with a top rail and midrail or another form of fall protection.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  On or about June 21, 2017, employees were exposed to trip and fall hazards from the open side of a stairway with 16 risers leading to the second floor and the stairway was not equipped with a stairrail system or handrail on the unprotected side.
