On or about 6/23/2017, a) An employee was not protected from a falling when working on the decking platform of the formwork that was approximately 12 feet above the level below. The employee fell when the formwork collapsed.

Location: 646 39th Street, Brooklyn, NY. On or about 6/23/2017, a) Formwork was not erected and braced as designed. An employee conducting work on the decking platform of the formwork was injured when the formwork collapsed.
