The scaffold platform was not designed and constructed by a qualified person in that the platform was not secured to the forklift. The employees were exposed to an approximate 18 foot fall to the ground below. b) Jobsite: On June 22, 2017 and at times prior thereto employees performed framing activities from a scaffold platform that was supported by a rough terrain forklift. The scaffold platform was not designed and constructed by a qualified person in that a guardrail system or other fall protection system was not in place. The employees were exposed to a 18 foot fall to the ground below. c) Jobsite: On June 22, 2017 and at times prior thereto employees performed framing activities from a scaffold platform that was supported by a rough terrain forklift. The scaffold platform was not designed and constructed by a qualified person in that the manufacturer warned of a personal injury hazard, prohibiting use of the machine as a personal lift, or to fit the machine with any form of personnel work platform.
