29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design:   a) Jobsite: On June 22, 2017 and at times prior thereto employees performed framing activities from a scaffold platform that was supported by a rough terrain forklift.  The scaffold platform was not designed and constructed by a qualified person in that the platform was not secured to the forklift. The employees were exposed to an approximate 18 foot fall to the ground below.  b) Jobsite: On June 22, 2017 and at times prior thereto employees performed framing activities from a scaffold platform that was supported by a rough terrain forklift.  The scaffold platform was not designed and constructed by a qualified person in that a guardrail system or other fall protection system was not in place.  The employees were exposed to a 18 foot fall to the ground below.  c) Jobsite: On June 22, 2017 and at times prior thereto employees performed framing activities from a scaffold platform that was supported by a rough terrain forklift.  The scaffold platform was not designed and constructed by a qualified person in that the manufacturer warned of a personal injury hazard, prohibiting use of the machine as a personal lift, or to fit the machine with any form of personnel work platform.  The employees were exposed to a 18 foot fall to the ground below.  Note) Abatement and Certification of corrective action ARE required for this item.
