29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:    (a) Big Sky Alignment and Brake Company: On or about June 22, 2017 and at times prior, employees were exposed to a fall hazard of approximately 11 feet while engaged in roofing activities on a 2:12 sloped commercial roof.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grab rail, provided to assist the employee in mounting and dismounting the ladder:  (a) Big Sky Alignment and Brake Company: On or about June 22, 2017 and at times prior, employees were exposed to a fall hazard of approximately 11 feet while accessing/egressing the unsecured Werner ladder.
