29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:   On 06/16/17, an employee's left hand ring and middle fingers were amputated while loading steel scrap metals into a flatbed truck in the parking lot.  The employer transported the injured employee to their emergency clinic and then to Gwinnett Medical Center for treatment.  The employer did not report within 24 hours as required until 06/19/17 when it was reported by the employee's mother.
