29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system: (a) On roof of Building 100 at the Grand Point Apartments in Poughkeepsie, NY - On or about June 26, 2017, employees were replacing the roof on the apartments with no fall protection. Employees were working on a roof approximately 40' above the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On roof of Building 100 at the Grand Point Apartments in Poughkeepsie, NY - On or about June 26, 2017, employees were accessing the roof on the apartments using a metal extension ladder that was only 1.5 feet above the roof surface.
