29 CFR 1926.1053(b)(6): Ladders shall be used on stable and level surfaces unless secured to prevent accidental displacement:  Jobsite:  The feet of an extension ladder from which an employee was painting the porch were set up on a pachysandra patch without appropriately secured to prevent displacement  On or about 06/14/2017, the above ladder was displaced while the employee was performing the task, causing the employee to fall and sustained a broken left tibia injury from the incident.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Jobsite:  The employee who was working from an extension ladder was not trained by a competent person in the proper procedures to minimize the hazards associated with ladders.
