On June 26, 2017, an employee was observed performing set-up and/or die changes and adjustments on the press brake. Hazardous Energy Control Procedures were not developed for this activities. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On June 26, 2017 an employee was observed performing setup operations on the Cincinnati Press Brake without the use of energy control procedures. On June 26, 2017 the operator was observed bending a part with his right hand next to the die. (b) Production area - Amada 80-25 SL Press Brake, was not equipped with guards or guarding devices to prevent the operator from inadvertently placing their hands or finger in the point of operation during the operating cycle. On June 26, 2017 the operator was observed bending parts with both hands directly adjacent to the die. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
