29 CFR 1915.81(a)(5): The employer shall dispose of flammable and combustible substances, such as paint thinners, solvents, rags, scrap, and waste, or store them in covered fire-resistant containers at the end of each workshift or when the job is completed, whichever occurs first.   Shipyard, near yacht "Call Me the Breeze":  Paint residue and solvent (i.e. Pettite thinner) soaked rags were disposed of in an open, blue plastic barrel with combustible materials such as cardboard and paper creating a fire hazard.

29 CFR 1915.152(a): The employer did not provide or ensure each affected employee with the appropriate personal protective equipment (PPE) for the eyes, face, head, extremities, torso, and respiratory system, including protective clothing, protective barriers, personal fall protection equipment, and life saving equipment, meeting the applicable provisions of Subpart I, wherever employees were exposed to work activity hazards that required the use of PPE.   Shipyard, docks, and aboard floating vessels:  Employees do not wear Coast Guard approved PFD's when working over water (marine lift or docks), within three feet of the waters' edge, and when operating vessels on waterways.  In the event that an employee falls in the water, a PFD would provide protection from drowning.

29 CFR 1915.152(b): The employer did not assess its work activity to determine whether there were hazards present, which necessitate the employee's use of personal protective equipment.       Shipyard:  Employees are required to wear various types of PPE such as but not limited to respirators, protective body, hand, face, head and foot wear when repairing or painting vessels.  A written hazard assessment has not been performed to include the elements listed below.  Additionally, the employer must evaluate tasks performed over water or near the waters' edge presenting drowning hazards and require wearing PFD's.       ABATEMENT NOTE:     1.     Select the type of PPE that will protect the affected employee(s) from the hazards identified in the occupational hazard assessment;    2.    Communicate selection decisions to affected employees;    3.    Verify that the required occupational hazard assessment has been performed through a document that contains  the following information: occupation, the date of the hazard assessment, and the name of the person performing the hazard assessment.

29 CFR 1915.158(b)(4): 90 feet of line was not attached to each ring life buoy.   Shipyard, docks and pier:  The lines attached to two ring life buoys were less than 90 feet in length.  The lines were approximately 60 feet long.
