On or before June 26, 2017, where employees mixed mortar containing Portland Cement, a suitable eye wash was not available, thereby exposing employees to contact hazards.

29 CFR 1926.51(f)(1): The employer shall provide adequate washing facilities for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be in near proximity to the worksite and shall be so equipped as to enable employees to remove such substances. a. On or before June 26, 2017, where employees mixed mortar containing Portland Cement, there were no adequate washing facilities available, thereby exposing employees to contact hazards.

On or before June 26, 2017, where employees were laying block in the east courtyard, the employees were not protected from falling into the access shaft down to the basement, thereby being exposed to a 10 foot, 5 inch fall hazard.
