29 CFR 1926.403(b)(1)(iii): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: Electrical insulation. The employer did not ensure that the insulation on the yellow AWG 12/3 extension cord that was plugged into the generator and providing power to a tool being used in the lift was free from damage. The cord was curled which presented signs of having been overloaded and the cord had the outer insulation damaged which resulted in the interior insulated conductors being exposed.
