Employee/s were exposed to a fall hazard of approximately twenty-seven (27) feet from the eave to the ground.

The employer did not ensure that employees utilized fall protection while performing siding installation on the 5/12 pitch roof of the front building. Employees were exposed to a fall hazard of approximately twenty-seven (27) feet from the eave to the ground.  b. The employer did not ensure that employees utilized fall protection while performing siding installation on the balcony of the front building, . Employees were exposed to a fall hazard of approximately nine feet (9) feet from the balcony floor to the ground.

29 CFR 1926.403(b)(1)(iii): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations:  Electrical insulation.  The employer did not ensure that the insulation on the yellow AWG 12/3 extension cord that was plugged into the generator and providing power to a tool being used in the lift was free from damage. The cord was curled which presented signs of having been overloaded and the cord had the outer insulation damaged which resulted in the interior insulated conductors being exposed.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  The employer did not ensure that employees were adequately trained to recognize the fall hazards they were exposed to and the appropriate procedures to protect themselves from falling.
