29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about June 26, 2017, an employee working in a trench greater than 5 feet deep was exposed to cave-in hazards. Adequate cave-in protection was not utilized. Abatement documentation is required for this item in accordance with 1903(d).
