29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219(f)(3): Sprockets and chains at the back of the Double Edge Trim Saw were unguarded. On or about June 28 2017, and at times prior thereto, the employer did not provide guards for belts, gears, shafts, and other moving parts on the following machines. a. In the Double Notcher Area, on the Morgan Double Notcher the sprockets and chain systems on the in feed side were not guarded. b. In the Trim Rip Line Area, on the Newman Km-16 Trim Rip Saw the in feed shaft and chain systems were not guarded. c. In the Trim Re-Saw Line Area, on the Newman KM-16 Trim Re-Saw the in feed shaft and chain system were not guarded. d. In the Trim Rip Line Area, on the Wood In Feed Conveyor System the two sprocket and chain systems were not guarded.

29 CFR 1910.213(r)(3): The point of operation on combination or universal woodworking machines was not guarded: On or about June 26 2017, and at times prior thereto, the cutting blades on the Morgan Double Notcher machine were not equipped with adequate guards to prevent employees' hands from reaching into the blades, exposing employees to the hazards of being struck by the blades.
