29 CFR 1910.179(b)(6)(ii): Where passageways or walkways are provided obstructions shall not be placed so that safety of personnel will be jeopardized by movements of the crane. On or about June 20, 2017, the semigantry crane, known as the bay-to-bay crane, positioned near the walkway of the band saw table created an obstruction to the movement of the overhead cab-operated crane, known as the stacker crane, jeopardizing the safety of the band saw operator.

29 CFR 1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard: On or about June 20, 2017, the employer did not conduct frequent (daily to monthly) inspections of cranes, including, but not limited to, the overhead cab-operated crane, known as the stacker crane, and a semigantry crane, known as the bay-to-bay crane. The cranes are used on a daily basis to move and load material throughout the facility.

29 CFR 1910.179(l)(3)(i): Any unsafe conditions disclosed by the inspection requirements of paragraph (j) of this section shall be corrected before operation of the crane is resumed. Adjustments and repairs shall be done only by designated personnel. On or about June 20, 2017, the employer did not ensure unsafe conditions disclosed during the periodic (annual) inspection [1910.179(j)(3)] of the overhead cab-operated crane, known as the stacker crane, were corrected before of operation of the crane resumed. The unsafe conditions included, but not limited to, the warning device lights and horn needed repair.
