(a) On or about 1/4/2018, the employer failed to submit documentation and certification of corrective action for citation 1, item 1 issued on 10/12/2017 with an abatement due date of 11/7/2017.  (b) On or about 1/4/2018, the employer failed to submit documentation and certification of corrective action for citation 1, item 2 issued on 10/12/2017 with an abatement due date of 11/7/2017.     (c) On or about 1/4/2018, the employer failed to submit documentation and certification of corrective action for citation 1, item 3 issued on 10/12/2017 with an abatement due date of 11/7/2017.
