29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:    a) Encoder Department - On or about 8/29/2017, employees were exposed to continuous noise at 84% of the permissible daily exposure of 85 dBA (8 hour time weighted average) or equivalent sound levels of 89 dBA during a 445 minute sampling period operating the Encoder Machine. The exposure calculation included a zero increment of the 35 minutes not sampled.     An effective hearing conservation program would include, initial baseline and annual audiograms, making hearing protection available, and initial and annual noise training.
