29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) For the single story house, as observed on or about 06/29/2017, employees were working on the roof and exposed to fall hazards of 9 feet and 11 inches. The roof pitch was 5:12. No fall protection was being used. Outstanding Construction Work, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1163087, citation number 1, item number 1 and was affirmed as a final order on 08/29/2016, with respect to a workplace located at 6950 Holly Heath Dr., Riverview, FL 33569.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) For the single story house, as observed on or about 07/14/2017, employees working on the roof at ground to eave heights of 9 feet and 11 inches were not trained on fall protection equipment or how to recognize fall hazards. Employees were not aware of the weight capacity requirements for anchor points or when fall protection was needed.
