29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel.  On or about June 2, 2017, at the Acrylonitrile Unit, the employer failed to perform adequate lock out on process Acid system piping and did not implement safe work practice to controls and prevent the hazardous release of stored (chemical) energy.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  On or about June 2, 2017, at the Acrylonitrile Unit, the employer failed to developed, document and utilize a procedure for the control of potentially hazardous energy when the employees were engaged in locking out the Acid system before contractors started working on the acid system.

On or about June 2, 2017, at the Acrylonitrile Unit, the employer failed to ensure that all potential and stored energy within the acid system piping was drained, disconnected, or otherwise rendered safe prior to a contractor crew performing line-breaking operations.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished.    On or about June 1, 2017, at the Acrylonitrile Unit, the employer failed to verify that the acid system piping had been adequately drained and relieved of stored energy (sulfuric acid) prior to the contractor crew working on the acid system.
