29 CFR 1926.403(b)(1): The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: Classification by type, size, voltage, current capacity, specific use. At the rear of the back building the yellow AWG 12/3 extension cord that was plugged into the generator and providing power to three AWG 16/3 extension cords. The cord was curled - presenting signs of having been overloaded. Employees were exposed to the hazard of contacting a damaged cord which may present a burn and/or shock hazard.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At the rear of the back building the black & red AWG 12/3 extension cord that was plugged into the generator did not have the male plug attached to the cord in a manner that ensured strain would not be applied directly to the screws within the plug.
