29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) North side of house on roof - On or about June 28, 2017 employees applying roof shingles on 3rd floor roof were not protected from falls with the use of personal fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) North side of house on second floor roof - On or about June 28, 2017 employees gained access to the upper roof using a 6 foot step ladder that did not extend 3 feet past the edge of the roof exposing employees to falls of 20 feet.
