The atmosphere in the lift station where work was taking place could become oxygen deficient, exposing employees to asphyxiation hazards.

Employees were not trained in required permit documentation measures, that atmospheric testing was needed when entering and while in the lift station, the need to have an attendant outside the space to help the entrant in an emergency situation, and employees were not trained on rescue procedures in the event of an emergency.
