29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.  a. In the Soap Pad Department, the employer did not ensure that Soap Pad Machine # 11 was shut down prior to allowing employees to place their hands inside the point of operation to clear a jam.   b. In the Soap Pad Department, the employer did not ensure that Soap Pad Machine # 12 was shut down prior to allowing employees to place their hands inside the point of operation to clear a jam.  c. In the Brillo Pack area, the employer did not ensure that the Tisma Cartoning machine (Serial # 1579) was shut down prior to allowing employees to place their hands inside the point of operation to clear a jam.   d. In the Fab area, the employer did not ensure that the Tisma Cartoning machine (Serial # 1412) was shut down prior to allowing employees to place their hands inside the point of operation to clear a jam.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. In the Brillo Pack area, the rotary area of the Tisma Cartoning machine (Serial #1579) was not guarded.    b. In the Soap Pad area, the left side of Soap Pad Machines 1-10 where employees use a tool to remove jams, were not adequately guarded.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    a. In the Fab area, a belt and pulley near the check weigher on the Fab Tisma Cartoning machine (Serial # 1412) was not guarded.
