29 CFR 1926.652(c): Support systems, shield systems, and other protective systems were not designed and constructed in accordance with the requirements of paragraph (c)(1-4); as applicable.    a) East End of Trench, Williamsport, PA - Employees installed piping from within a trench while ensconced within an inadequate "home-made" wooden trench shield that was not designed by a professional engineer and without documented tabulated data identifying the specifications, recommendations, and limitations of the protective system exposing the employees to struck-by and caught-in hazards in the event of a cave-in, on or about June 28, 2017.    In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.    a) East End of Trench, Williamsport, PA - Employees worked within a (4-5 ft W x 5-5'6 ft D x 48 ft L) excavation without a safe means of egress. Employees were using the a dirt ramp located at the end of the excavation for access & egress, as observed on or about June 27, 2017.
