29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded:  (a) On June 2017, at the facility located at 6100 Stilwell St, Kansas City, MO., employees were exposed to serious injuries to their hands and or fingers, in that the point of operation on the End Line Hydraulic shear was not guarded. As a result, one employee received amputation injuries to finger tips on his left hand while attempting to retrieve a sample metal piece that had been cut in the point of operation of the shear.
