On or about June 29, 2017 at 2961 Sussex Road in Green Bay, Wisconsin, the employer did not conduct adequate safety inspections prior to employees working on a roof surface 17 ft. above surface level. No fall protection equipment installed for work on roof surface, unguarded hole in roof surface, improper use of step ladder, and no ladder provided for 3 ft. break in elevation at roof access point.
