29 CFR 1926.20(b)(2): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with accident prevention; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about June 29, 2017 at 2961 Sussex Road in Green Bay, Wisconsin, the employer did not conduct adequate safety inspections prior to employees working on a roof surface 17 ft. above surface level.  No fall protection equipment installed for work on roof surface, unguarded hole in roof surface, improper use of step ladder, and no ladder provided for 3 ft. break in elevation at roof access point.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about June 29, 2017 at 2961 Sussex Road in Green Bay, Wisconsin, employees were installing roof sheathing at 11 ft. above surface level with an unprotected hole in the roof surface.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  On or about June 29, 2017 at 2961 Sussex Road in Green Bay, Wisconsin, the employer failed to provide required written operator training certification for employees operating a SkyTrak 8042 Telehandler forklift.
