29 CFR 1910.147(c)(4)(ii)(A):  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to a specific statement of the intended use of the procedure;   a)  On or about June 30, 2017 on the Grated Line 1 at W4200 Co Hwy PP in Plymouth, the employer did not include the task of clearing jams in the scope of the Lockout Tag Out program for the North Blender.  Employees were clearing jams at the discharge door without exclusive control of hazardous motion which exposes the employees to crushing hazards.  b)  On or about June 30, 2017 on the Grated Line 1 at W4200 Co Hwy PP in Plymouth, the employer did not include the task of clearing jams in the scope of the Lockout Tag Out program for the South Blender.  Employees were clearing jams at the discharge door without exclusive control of hazardous motion which exposes the employees to crushing hazards.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about June 25, 2017 on the Grated Line 1 at W4200 Hwy PP in Plymouth, Wisconsin, the employer did not conduct periodic inspections of the energy control procedures to ensure an accurate procedure and employees were familiar with their responsibilities under the procedures.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  On or about June 30, 2017 on the Grated Line 1 at W4200 Hwy PP in Plymouth, Wisconsin, authorized employees who perform clearing of jams on the north and south blenders had not received training to unjam the blenders.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a)  On or about June 30, 2017 at W4200 Co Hwy PP in Plymouth, Wisconsin, employees were operating the North Blender within the Grated Line 1 without adequate guarding.  Employees were able to touch the blender paddles by reaching around the guard and into the product discharge opening.  b)  On or about June 30, 2017 at W4200 Co Hwy PP in Plymouth, Wisconsin, employees were operating the South Blender within the Grated Line 1 without adequate guarding.  Employees were able to touch the blender paddles by reaching around the guard and into the product discharge opening.

29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available.  On or about June 25, 2017 at W4200 Co Hwy PP in Plymouth Wisconsin, the employer did not provide a trained first aid responder for third shift employees who were clearing product jams and resetting machine defaults on the Grated Line 1.
