29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million (ppm): a. At the worksite - an employee was exposed to carbon monoxide (CO) during a 240 minute occupational exposure at a mean calculated 252.3 ppm CO level (SAE = 0.182). The %COHb in the employee's blood reached a calculated peak level of 23.0%. Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account. The 8-hour TWA exposure was 126.2 ppm CO (SAE = 0.184), which is 2.52 times the OSHA PEL of 50 ppm CO. Violation observed on or about June 19, 2017. b. An employee was exposed to carbon monoxide (CO) during a 240 minute occupational exposure at a mean calculated 263.8 ppm CO level (SAE = 0.268). The %COHb in the employee's blood reached a calculated peak level of 27.3%. Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account. The 8-hour TWA exposure was 131.9 ppm CO (SAE = 0.270), which is 2.64 times the OSHA PEL of 50 ppm CO. Violation observed on or about June 19, 2017. Time Weighted Average (TWA) exposure was averaged over an 8-hour shift - zero exposure was assumed for the time not sampled for each calculation. Violation observed on or about June 19, 2017.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): a) In the warehouse and refrigerated areas - employees were exposed to carbon monoxide (CO) while working around a Hyster LPG Forklift, model S35XM, serial number C010H01526S, that was being used inside the work area. Employees became ill and the fire department and emergency medical services were called, where subsequent evaluation determined the employees had CO overexposure. Administrative or engineering controls were not implemented to ensure CO levels did not exceed the OSHA PEL. An employee was exposed to carbon monoxide (CO) during a 240 minute occupational exposure at a mean calculated 252.3 ppm CO level (SAE = 0.182).
