29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about June 29, 2017 at or near 5700 Pierce Manse, Benton, AR 72019, the employer exposed employees operating nail guns without the use of eye protection at a residential roofing worksite to hazards of flying projectiles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 29, 2017 at or near 5700 Pierce Manse, Benton, AR 72019, the employer exposed employees working above 6ft to fall from height hazards when removing and installing shingles on a 10/12 pitched roof.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1115596, citation number 1, item number 1 and was affirmed as a final order on March 15, 2016, with respect to a workplace located at 2006 Breckenridge Drive, Little Rock, AR. 72227.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about June 29, 2017 at or near 5700 Pierce Manse, Benton, AR 72019, the employer allowed employees to use ladders that were only extended approximately 0.5 foot above a 10/12 pitched roof surface they were installing shingles on.  The Gonzalo Garrido-Cortes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1115596, citation number 1, item number 2a and was affirmed as a final order on March 15, 2016, with respect to a workplace located at 2006 Breckenridge Dr, Little Rock, AR 72227
