29 CFR 1910.119(d)(3)(ii): The employer shall document that equipment complies with recognized and generally accepted good engineering practices. a) On or about July 3, 2017, the supply valve and the vacuum valve for the 3 Ton Container and 2 Cylinder Filling stations used a wrench to open, adjust and close each valve. Using wrenches does not provide a positive visual indication of the valve position since the wrenches can be placed on the valves in different positions. The supply valve and the vacuum valve for the 3 Ton Container and 2 Cylinder Filling stations are the same valves used for chlorine ton containers. Usage of ton container valves in a stationary piping system does not provide positive valve position indication and is not recommended by The Chlorine Institute March 2013 Pamphlet 6 Section 5.4. b) On or about July 24, 2017, the following Chlorine Header instruments were not identified with tags or other means: TT-100, TE-101, PG-310, PIT-310 and PI-310A. Instrument identification is necessary to document instrument inspections as recommended by The Chlorine Institute May 2011 Pamphlet 17 Section 2.4.b.

29 CFR 1910.119(f)(1)(iii)(B): The employer shall develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and shall address at least the following elements: Precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment. a) Chlorine Railcar Unloading - Packaging Department Standard Operating Procedure Number P6011OS, used on July 3, 2017, did not indicate two chlorine rail cars are connected to the chlorine supply system that provides chlorine to the Chlorine Cylinder Filling Stations and Powell Bleach Plant. b) Chlorine Railcar Unloading - Packaging Department Standard Operating Procedure Number P6011OS, used on July 3, 2017, did not include engineering or administrative controls to prevent one chlorine railcar charging the chlorine supply system before the other chlorine railcar hook-up procedures have been completed.
