29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about June 30, 2017, during the replacement of a residential sewer line for a construction project located at 2149 McKinley Avenue, Lakewood, Ohio, the employer did not ensure employees working inside a trench, with a varying depth of approximately 6.5 to 7 feet and with near 90 degree vertical walls, were protected from cave-ins by an adequately designed protection system. The soil would have been classified as; Cohesive Soil, type B .
