29 CFR 1926.501(b)(14):  Each employee working on, at, above, or near wall openings, where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was  less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:   The employer is failing to protect employees from fall hazards associated with performing construction activity while exposed to falls 6 feet or greater.  The most recent occurrence of this was observed on June 29, 2017 at the new apartment complex being built at 3625 Dewey St. Omaha, NE where the employer has a crew of 2 employees setting patio doors into framed out wall openings without any form of fall protection system.
